Abstract. A 75 years old woman was diagnosed with breast cancer and lymph node metastasis. The breast tumour was diagnosed as a lobular breast cancer with 100% oestrogen receptor expression and HER2 negative. In the axilla a lymph node metastasis was proved but there was no immunohistochemistry information of this metastasis. The patient underwent a [18F]FDG PET /low-dose CT for staging. This PET scan revealed a very low maximum standardized uptake value (SUVmax) of 1.9 in the breast tumour, which is commonly seen in patients with a lobular breast cancer, but there was a very high SUVmax of 9,6 in the axillary metastasis.
